Monmouth Family Health Center is a deemed Federal Public Health Service and receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals. The center is also licensed by the NJ Department of Health and Senior Services and is also an FTCA deemed facility.
Directions to our 270 Broadway Location:
FROM NORTH NJ – From the North, take NJ Turnpike South to Exit 11 for the Garden State Parkway South. Head south and take exit 105. When you exit stay on Route 36 eastbound for about 5 miles. Turn right onto Broadway, after approximately 2.5 miles, 270 Broadway will be on your right.
FROM SOUTH NJ – From the South, take the Garden State Parkway North to Exit 105. Then follow directions above.
FROM WEST NJ – From the West, take State Route 195 East to the Garden State Parkway North exit.
Head north and take exit 105. Then follow the directions above.
From Route 18 and from the Garden State parkway southbound AND northbound.
Directions to our 80 Pavilion Ave Location:
FROM NORTH NJ – Take NJ Turnpike to Exit 11 South to Garden State Parkway South. Travel 23 miles to Exit 105, stay on route 36 East for 3.4 miles then turn right onto Broadway. Travel 2.3 miles to make a right onto Ocean Ave. After 0.5 miles, make a right onto Pavilion Ave, we are located on the following cross street.
FROM SOUTH NJ – Take Garden State Parkway North. Exit 105, then follow the directions above.
FROM WEST NJ – Take Route 195 East to the Garden State Parkway North. Exit 105, then follow the directions above.
From Route 18 and from the Garden State parkway southbound AND northbound.
Directions to our 335 Broadway Location:
FROM NORTH – Take NJ Turnpike to Exit 11 South to Garden State Parkway South. Head south and take exit 105. Stay on Route 36 Eastbound for approximately 3.4 miles and turn right onto Broadway. Travel 1.6 miles to 335 Broadway on your left.
FROM SOUTH – Take Garden State Parkway North. Exit 105, then follow the directions above.
FROM WEST – Take Route 195 East to the Garden State Parkway North. Exit 105, then follow the directions above.
